ABSTRACT

Objective:To investigate the prevalence of malnutrition in hospitalized patients with ulcerative colitis (UC) using the Global Leadership Initiative on Malnutrition (GLIM) criteria, compare the body composition, clinical indicators, and occurrence of poor UC outcomes between the malnutrition group and the non-malnutrition group, and analyze the risk factors of malnutrition.Methods:A total of 104 UC patients hospitalized in the Affiliated Hospital of Qingdao University during 2019 to 2021 and with relevant data collected through UC-sarcopenia Prospective Study were included. Data including nutritional risk screening 2002 (NRS 2002) scores, diagnosis of malnutrition per GLIM criteria, body composition data, clinical data and occurrence of poor outcomes were extracted. All patients were screened for nutritional risk using NRS 2002 at admission, and those at nutritional risk were further evaluated for malnutrition diagnosis per GLIM criteria. The prevalence of malnutrition in UC inpatients, the differences in indicators between malnutrition and non-malnutrition groups, and the risk factors of malnutrition were analyzed.Results:Among the enrolled UC patients , 35 (33.7%) were at nutritional risk, and 30 (28.8%) were diagnosed as malnutrition by GLIM criteria. Compared with the non-malnutrition group, the body composition indexes, including body fat mass, protein, skeletal muscle mass, segmental muscle mass, body water, waist circumference, arm circumference, visceral fat area, basal metabolic rate and body cell mass, all decreased to some extent in the malnutrition group ( P < 0.05). Prealbumin and albumin levels were lower ( P < 0.01). The modified Mayo endoscopic score (MMES), C-reactive protein level and erythrocyte sedimentation rate were higher ( P < 0.01). The rates of re-admission and surgery due to disease activity at 90 days were higher ( P < 0.05). High MMES ( OR =1.534, P = 0.044) and low albumin level ( OR =0.781, P = 0.013) were risk factors for malnutrition in UC patients. Conclusions:GLIM criteria is suitable for the diagnosis of malnutrition in hospitalized UC patients, and malnutrition patients are more likely to experience poor outcomes such as short-term re-admission and surgery. UC patients with higher MMES and lower albumin level are more likely to suffer from malnutrition.

ABSTRACT

ObjectiveTo study the effects of Toddalia asiatica alcohol extract on autophagy and apoptosis of non-small cell lung cancer A549 cells, and to explore its possible mechanism. MethodA549 cells were cultured in vitro. Cell counting kit-8 (CCK-8) was used to detect the proliferation of A549 cells, and cell survival rate was calculated to screen the drug concentration. The apoptosis in each dose group and that after the use of 3-methyladenine (3-MA), an autophagy inhibitor, were detected by flow cytometry combined with Annexin V-FITC/PI double staining. Western blot was used to detect the expression levels of apoptosis-related proteins such as B cell lymphocytoma-2(Bcl-2), Bcl-2-associated X protein(Bax), microtubule-associated protein 1 light chain 3 (LC3), cleaved cysteinyl aspartate-specific protease-3 (cleaved Caspase-3), activated poly (Adenosine diphosphate) ribonucleotide polymerase (cleaved PARP1), PARP1, activated death activator (t-Bid), Bid, and ubiquitin-binding protein p62 in each group and those after the use of 3-MA. ResultCompared with the conditions in the control group, the cell survival rate in 0.25 g·L-1 group (P<0.05), and 0.5, 1, 2, 4 g·L-1 groups (P<0.01) was decreased after 24 h intervention. Additionally, the cell survival rate was reduced in a concentration-dependent manner at 48 h and it was less than 10% at 4 g·L-1 (P<0.01). Compared with the conditions in the control group, the total apoptosis rate in 0.5 g·L-1 group was increased (P<0.05), and the apoptosis rate in 1 and 2 g·L-1 groups was also increased (P<0.01). Compared with the 2 g·L-1 group and 3-MA group, the 3-MA combined with T. asiatica alcohol extract had significantly decreased apoptosis rate (P<0.01). Compared with the conditions in the control group, elevated expression of pro-apoptotic proteins cleaved PARP1, Bax and t-Bid in 1 and 2 g·L-1 groups (P<0.05, P<0.01), and reduced expression of Bid in the 2 g·L-1 group (P<0.01) were found. Compared with the conditions in the control group, the expression of anti-apoptotic protein Bcl-2 (P<0.05, P<0.01) and the level of p62 (P<0.01) were down-regulated in 0.5, 1, 2 g·L-1 groups, while the level of LC3 Ⅱ protein was up-regulated (P<0.01), with certain concentration dependence. ConclusionT. asiatica alcohol extract could significantly inhibit the proliferation of A549 cells, which might be related to promoting autophagy and inducing apoptosis.

ABSTRACT

Objective:To explore the risk factors of hypotension in patients with hemodialysis-related superior vena cava diseases.Methods:This was a retrospective cohort study. The maintenance hemodialysis patients diagnosed as superior vena cava stenosis ≥50% or occlusion in West China Hospital of Sichuan University from January 1, 2019 to March 31, 2022 were selected. The patients were divided into hypotensive group and non-hypotensive group according to the occurrence of hypotension during non-dialysis period. The differences of general clinical data, cardiac ultrasound parameters and blood biochemical indexes between the two groups were compared. Multivariate logistic regression model was used to analyze the risk factors of hypotension in patients with hemodialysis-related superior vena cava lesions.Results:A total of 202 patients with hemodialysis-related superior vena cava lesions were included, of whom 84 patients (41.6%) developed hypotension during non-dialysis. Compared with the non-hypotensive group, patients in the hypotensive group had longer dialysis age ( Z=2.093, P=0.036), shorter left atrial diameter ( t=2.316, P=0.022), lower aortic valve orifice flow rate ( t=3.702, P=0.001) and serum calcium ( t=2.320, P=0.021), thicker left ventricular posterior wall ( t=3.042, P=0.003), lower proportions of hypertension history ( χ2=9.125, P=0.003), and higher proportion of residual superior vena cava ≤30% or occlusion ( χ2=8.940, P=0.003) and azygous vein opening ( χ2=11.067, P=0.001). Multivariate logistic regression analysis results showed that no history of hypertension ( OR=0.383, 95% CI 0.156-0.943, P=0.037), slow aortic valve orifice flow ( OR=0.207, 95% CI 0.062-0.695, P=0.011) and low blood calcium ( OR=0.236, 95% CI 0.066-0.841, P=0.026) were the independent influencing factors of hypotension in patients with hemodialysis-related superior vena cava diseases. Conclusions:Hemodialysis patients with severe stenosis or occlusion of the superior vena cava during non-dialysis have a higher risk of hypotension. No history of hypertension, slow aortic valve orifice flow, and low blood calcium are the independent risk factors of hypotension in patients with hemodialysis-related superior vena cava diseases.

ABSTRACT

Chronic obstructive pulmonary disease (COPD), a common clinical chronic respiratory disease, has a long course and is intractable. It is closely related to many factors, such as immune imbalance. Helper T cell 17 (Th17), an immune-promoting cell, and regulatory T cell (Treg), an immunosuppressive cell, maintain the balance of the immune microenvironment together. In the course of COPD, the proportion of Th17 cells usually increases, while the proportion of Tregs that inhibit Th17 activity decreases. Their coordination and balance are critical in the inflammatory and immune processes of COPD. At present, COPD is mainly treated with nasal inhalation preparations and oral drugs by western medicine. In spite of a certain therapeutic effect, side effects of drugs and heavy economic burden are becoming increasingly prominent. Modern research shows that traditional Chinese medicine (TCM) has the characteristics of few side effects, stable curative effect, and multi-target regulation, and it is advantageous and promising in the prevention and treatment of COPD. In recent years, a large number of TCM clinical and experimental trials on the intervention of Th17/Treg balance in COPD have been launched. Substantial pieces of evidence confirm that the intervention of Th17/Treg balance is an important potential target of TCM in the treatment of COPD. This study reviewed the previous research on the intervention effect of single Chinese medicine, effective components of Chinese medicine, and Chinese medicinal compound on Th17/Treg balance in COPD to comprehensively reveal the potential target of Th17/Treg balance in COPD for clinicians and scientific researchers, promote relevant research, and provide references for the rational application of TCM in the prevention and treatment of COPD.

OBJECTIVE@#To assess the association of JAG2 gene single nucleotide polymorphisms with the occurrence of nonsyndromic cleft lip with or without cleft palate (NSCLP) among northwest Chinese population.@*METHODS@#A case-control study was carried out on 301 NSCLP patients and 304 healthy controls. An iMLDR(TM) genotyping technique was used to detect three single nucleotide polymorphisms (SNPs) [rs741859 (T/C), rs11621316 (A/G) and rs1057744(C/T)] of the JAG2 gene. Allelic and genotypic frequencies and haplotypic distribution among the two groups were compared.@*RESULTS@#A significant difference was found in the frequency of C and T alleles for rs741859 between the two groups. The CT genotype of rs741859 could significantly reduce the risk for NSCLP to 65% (P 0.8), whose distribution difference between the two groups was not statistically significant (P> 0.05).@*CONCLUSION@#The CT genotype of the JAG2 gene rs741859 may confer a protective effect for NSCLP among northwest Chinese population.

ABSTRACT

Objective:To further explore the self-knowledge and real experience of hemophilia patients in childbearing age, and provide evidence for nursing workers to design and implement personalized effective intervention strategies.Methods:A semi-structured in-depth interview was conducted among 10 hemophilia patients of hemophilia clinic in our hospital with the purpose sampling method. The data were analyzed by Colaizzi 7-step analysis.Results:Three major themes were refined: emotional experience (role change, fertility anxiety, lack of self-confidence in interpersonal relationships); lack of knowledge; disease benefit.Conclusion:Patients with hemophilia have diverse and complex real psychological experiences during childbearing age. Medical staff develop targeted and personalized health education strategies for patients at this stage to better prevent and treat them and improve their quality of life. Population quality.

ABSTRACT

Ischemic retinopathy,resulting in multiple lesions like microvasculature damage,inflammation and neovascularization,is a major contributor of vision damage.In these pathological changes,retinal glia cannot be ignored in the development of retinopathy.They constitute a highly versatile population that interacts with various cells to maintain homeostasis and limit disease.Therefore,glial activation and gliosis are strikingly ubiquitous responses to almost every form of retinal disease.Both of microglial cells and Müller cells are major intrinsic retinal glial cells and they are in close relationship,which means they can influence each other,make joint action or even become interdependent.They exhibit morphological and functional changes to have an impact on degree of retinal injury through different responses,which mediated by glial cells are important not only for course of disease progression,but also for the maintenance of neuronal and photoreceptor survival.Thus,defining the mechanisms that underlie communications between microglial cells and Miller cells could enable the development of more selective therapeutic targets,with great potential clinical applications.

ABSTRACT

Objective To investigate the temporal and spatial distribution characteristics of non-syndromic cleft lip with or without palate (NSCLP) who born in Gansu Province from 2010 to 2016, and to establish a predictive model for developing the strategies for the prevention and control of NSCLP. Methods Spatial epidemiological research method and geographical information systems (GIS) were used to conduct thematic mapping, spatial correlation analysis, high/low clustering analysis, hotspot analysis and Kirging interpolation prediction for NSCLP patients in Gansu Province from 2010 to 2016. Results From 2010 to 2016, the aggregation trend of NSCLP incidence in 89 counties in Gansu Province was different obviously, the southeast area was high and the northwest was low. Based on the data of the cumulative incidence of NSCLP from 2010 to 2016 in Gansu, the spatial distribution of NSCLP presented positive spatial correlation (Moran’I=0.274,Z=7.814,P<0.001) and the aggregation type was high-high cluster(Getis Gi=0.000 003,Z=4.381,P<0.001), with 22 hot spots. The Kirging interpolation prediction results showed that the main prevalence trend of NSCLP in Gansu extended from Longdong to Longxi and Longnan areas. Conclusions The geographical distribution of NSCLP had a positive spatial correlation and a high-high aggregation type in Gansu from 2010 to 2016. The high aggregation area is concentrated in Longdong, Longxi and Longnan of Gansu, which suggest that it is essential to focus on prevention and control in these areas.

ABSTRACT

Ziziphi Spinosae Semen (ZSS) has been used for treatment of insomnia in China for centuries. To reveal the influence of insomnia on the levels of the neurotransmitters including serotonin (5-HT), glutamic acid (Glu), γ-aminobutyric acid (GABA), noradrenaline (NE) and dopamine (DA), and to study the role of ZSS aqueous extract in the treatment of insomnia, an UPLC-ESI- MS/MS method was developed and validated for simultaneous determination of five neurotransmitters in the rat brain. The brain samples were pretreated by one-step direct protein precipitation with acetonitrile. The analytes were detected in positive mode with multiple reaction monitoring (MRM) and the procedure was completed in less than 10 min. The method showed a good linearity (R > 0.9967) with the other validation parameters were within acceptance range. The results indicated that the concentration of 5-HT, GABA and DA is significantly lower (P < 0.01) in para-chlorophenylalanine (PCPA)-induced insomnia rat model group, while Glu and NE significantly higher than those in control group (P < 0.01). Treatment with ZSS aqueous extract (4 or 8 g·kg·d for seven days) could ameliorate the symptoms of insomnia by significantly changing the levels of the neurotransmitter parameters mentioned above. The data obtained in this study demonstrate that ZSS aqueous extract could ameliorate the symptoms of insomnia by modulating the levels of monoamines and amino acid neurotransmitters in the brain.

Alzheimer's disease is a common neuro-degenerative disease.The clinical diagnosis mainly depends on the patient's complaint,the score of mini-mental state examination and Montreal cognitive assessment scale,and the comprehensive judgment of MRI and other imaging examinations.Retina is homologous to brain tissue,and their vascular systems have similar physiological characteristics to small blood vessels in the brain.Numerous studies found that the thickness of retinal nerve fiber layer,visual function,retinal blood vessels and retinal oxygen saturation were changed in AD patients to different degrees.To explore the formation mechanism and significance of ocular fundus changes in AD patients will be helpful to select specific,sensitive and simple methods for early observation and evaluation of AD.

ABSTRACT

Objective To investigate the effect of razumab combined with 25 G transconjunctival sutureless vitrectomy in patients with idiopathic macular epiretinal membrane (IMEM).Methods 57 cases (57 eyes) of IMEM patients in our hospital were divided into two groups according to random number table.28 cases in the control group were treated with 25G transconjunctival sutureless vitrectomy,and 29 cases in the observation group were treated with rezumab on the basis of the control group.The improvement of visual acuity,changes of macular fovea thickness (central foveal thickness,CFT),serum levels of vascular endothelial growth factor (VEGF) and intercellular adhesion molecule-1 (ICAM-1) in the two groups were observed and compared.The changes of intraocular pressure before treatment and at different time points (1 weeks,1 months,3 months) after treatment and incidence of complications in two groups were statistically analyzed.Results 3 months after treatment,the improvement rate of visual acuity in the observation group was 75.86% (22/29),higher than that in the control group 50% (14/28) (P < 0.05).The CFT of the two groups decreased after 3 months of treatment,and the CFT of the observation group was less than that of the control group after 3 months of treatment (P < 0.05).The serum levels of VEGF and ICAM-1 in the observation group were lower than those in the control group 1 week after treatment (P < 0.05).After 1 week of treatment,the intraocular pressure in the observation group was higher than that before treatment,but there was insignificant difference between the two groups.And there was insignificant difference in intraocular pressure between the two groups at 1 and 3 months after treatment (P > 0.05).The incidence of complications in the observation group was 10.34% (3/29),while that in the control group was 7.14% (2/28),with no significant difference between the two groups (P > 0.05).Conclusions The combination of rezumab and 25G transconjunctival sutureless vitrectomy can reduce the CFT of IMEM patients,improve their levels of serum ICAM-1 and VEGF,and improve their vision.Besides,it has little influence on intraocular pressure and fewer complications.

ABSTRACT

[Abstract] Objective To investigate the effects of exclusive or joint application of all-trans retinoic acid (ATRA) and bone morphogenetic proteins (BMPs) on the osteocalcin (OCN) in mouse osteogenic precursor cells (MC3T3-E1) and bone marrow mesenchymal stem cells (BMSCs), and on the expressions of osteogenesis-related genes ALP, OCN and Runx2. Methods MC3T3-E1 cells and BMSCs were subcultured and stimulated with different concentrations of cytokines, and then divided into ten groups: control group, ATRA group, 5ng/ml BMP2/7 group, 50ng/ml BMP2/7 group, 5ng/ml BMP2 group, 50ng/ml BMP2 group, ATRA+5ng/ml BMP2/7 group, ATRA+50ng/ml BMP2/7 group, ATRA+5ng/ml BMP2 group, and ATRA+50ng/ml BMP2 group. The morphology of the cells was observed by immunofluorescence staining. The OCN expressions in the two kinds of cells were detected by ELISA. The expressions of ALP, OCN and Runx2 genes were detected by RT-qPCR. Results Immunofluorescence staining showed that the cells grew well and were structurally intact. Exclusive use of ATRA obviously inhibited the expressions of OCN in both MC3T3-E1 and BMSCs. On the 7th day, for MC3T3-E1 cells: ATRA alone group [(20.97±0.31)ng/ml] significantly decreased than control group [(33.45±0.55)ng/ml]; for BMSCs: ATRA alone group [(9.90±0.16)ng/ml] significantly decreased than control group [(14.30±0.53)ng/ml] (P<0.05). Exclusive application of BMPs promoted OCN expression in a concentration-dependent manner, and BMP2/7 showed a more strong promoting role. The OCN content in MC3T3-E1 cells treated with 50ng/ml BMP2/7 was (47.13±0.59)ng/ml, and in BMSCs was (49.92±0.53)ng/ml. 50ng/ml BMP2/7 completely antagonized the inhibitory effect of ATRA on cell OCN expression (P<0.05). RT-qPCR test showed that, for MC3T3-E1: ATRA alone significantly inhibited the expressions of ALP and OCN gene (P<0.05), but promoted the expression of Runx2 gene (P<0.05). BMPs alone promoted the expression of osteogenesis-related genes in a concentration-dependent manner (P<0.05). ATRA significantly inhibited the promoting role of BMPs on the OCN gene expression, but ATRA and 50ng/ml BMP2/7 synergistically promoted the expression of ALP gene (P<0.05); for BMSCs: ATRA alone significantly promoted the expression of Runx2 gene on the 7th day, but inhibited the expressions of ALP and OCN gene (P<0.05). 50ng/ml BMPs alone significantly promoted the expressions of ALP and Runx2 genes (P<0.05). 50ng/ml BMP2/7 antagonized the inhibitory effect of ATRA on ALP gene expression and promoted its expression (P<0.05). Conclusions ATRA alone may inhibit the expression of OCN in cells, and inhibits the expressions of ALP and OCN genes in cells; and BMP2/7 may significantly promote the expression of OCN in cells, and may promote the expression of osteogenesis-related genes; and 50ng/ml BMP2/7 may completely antagonize the osteogenic inhibition of ATRA on cells.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the association between intestinal bifidobacteria and allergic diseases in infants by comparing the composition of intestinal bifidobacteria between healthy infants and infants with allergic diseases.</p><p><b>METHODS</b>A total of 48 infants were enrolled, and fecal samples were collected on days 0, 2, 7, and 15 and at months 1, 6, and 12 after birth. Among these infants, 22 who experienced allergic diseases before the age of 1 year were enrolled as allergic group and 26 healthy infants were enrolled as healthy group. Quantitative real-time PCR was used for the qualitative and quantitative analyses of Bifidobacterium and 8 species of bifidobacteria in fecal samples.</p><p><b>RESULTS</b>There was a difference in the composition of intestinal bifidobacteria between the two groups within 1 month after birth: the healthy group showed a reduction in bifidobacteria on day 2, while this feature was not observed in the allergic group. Compared with the healthy group, the allergic group had a significantly lower detection count of Bifidobacterium at month 1 (P<0.05) and a significantly lower detection rate of B.breve on day 15 (P<0.05), with delayed colonization of B.infantis.</p><p><b>CONCLUSIONS</b>Intestinal bifidobacteria and their composition within 1 month after birth may be associated with the development of allergic diseases, and this period of time may be a critical period for the prevention and treatment of allergic diseases in infants.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of ceftriaxone on the intestinal epithelium and microbiota in mice in the early-life stage, as well as the recovery of the intestinal epithelium and reconstruction of intestinal microbiota in adult mice.</p><p><b>METHODS</b>A total of 36 BALB/C neonatal mice were randomly divided into control group and experimental group, with 18 mice in each group. The mice in the experimental group were given ceftriaxone 100 mg/kg every day by gavage within 21 days after birth. Those in the control group were given an equal volume of normal saline by gavage. Immunohistochemistry was used to measure the expression of Ki67, Muc2, and ZO-1 in the intestinal epithelium. qPCR and next-generation sequencing were used to analyze the overall concentration and composition of fecal bacteria.</p><p><b>RESULTS</b>After 21 days of ceftriaxone intervention, the experimental group had a significant reduction in body weight, a significant reduction in the expression of Ki67 and ZO-1 and a significant increase in the expression of Muc2 in intestinal epithelial cells, a significant reduction in the overall concentration of fecal bacteria, and a significant increase in the diversity of fecal bacteria compared with the control group (P<0.05). Firmicutes was the most common type of fecal bacteria in the experimental group, and there were large amounts of Staphylococcus and Enterococcus. The experimental group had a certain degree of recovery of the intestinal epithelium, but there were still significant differences in body weight and the structure of intestinal microbiota between the two groups at 56 days after birth (P<0.05).</p><p><b>CONCLUSIONS</b>Early ceftriaxone intervention significantly affects the development of the intestinal epithelium and the construction of intestinal microbiota in the early-life stage. The injury of the intestinal microbiota in the early-life stage may continue to the adult stage and affect growth and development and physiological metabolism.</p>

ABSTRACT

OBJECTIVE:To provide reference for strengthening the supervision and management of responsibility of pharmaceutical corporates in China.METHODS:The status quo of social responsibility of pharmaceutical corporates in China were analyzed,and international experience of the United States,European countries and so on were analyzed.The ways to enhance the social responsibility of pharmaceutical corporates in China were proposed.RESULTS & CONCLUSIONS:Since the corporate social responsibility was first written into the People's Republic of China Company Law in 2005,the Chinese government has attached great importance to the implement of corporate social responsibility;however,the social responsibility of pharmaceutical corporates in China is still in the initial stage and lags behind other industries.The United States promotes corporate social responsibility through the formulation of perfect medical policies and regulations,non-governmental organizations participating in supervision,and the establishment of a sound financial incentive mechanism;Government in Germany,Britain and other European countries play an important role in promoting corporate social responsibility;at the same time,perfect supervision system of civil society is an important driving force to promote corporate social responsibility.When promoting the social responsibility of pharmaceutical corporates in China,it is necessary to improve the awareness of social responsibility of pharmaceutical enterprises through multiple paths,such as giving full play the supervision and management role of government,strengthening public supervision and management function,strengthening the system construction of social financial responsibility,improving the awareness of responsibility of pharmaceutical corporates.

ABSTRACT

·AIM:To research the correlation between dry eye and diabetic retinopathy (DR) in type 2 diabetes mellitus patients. ·METHODS:A total of 150 patients with type 2 diabetes mellitus collected from August 2015 to February 2017 were separated to three groups: without diabetic retinopathy ( DR ), with nonproliferative diabetic retinopathy ( NPDR) and with proliferative diabetic retinopathy (PDR). Each group contained 50 cases (100 eyes). Another 50 dry eye patients without diabetes were as control. All patients were provided with questionnaire survey of ocular surface disease index(OSDI),SchirmerⅠtest(SⅠt), break-up time (BUT) and fluorescein staining (FL), to compare the prevalence and the severity of dry eye in these groups. ·RESULTS:The prevalence of dry eye in type 2 diabetes mellitus patients without DR group was 44%, in type 2 diabetes mellitus patients with NPDR group was 51% and in type 2 diabetes mellitus patients with PDR group was 59%. To compare dry eye patients without DR with the control group, there were no difference in OSDI, SIt, BUT and FL(P>0.05). Dry eye in the patients with NPDR was more serious than in the patients without DR, there were statistical difference in OSDI,SⅠt,BUT and FL(P<0.05). Dry eye in the patients with PDR was more serious than in the patients with NPDR, there were statistical difference in SIt, BUT and FL (P<0.05). But OSDI was lower and showed no difference with patients without DR (P>0.05). ·CONCLUSION: The incidence and the severity of dry eye increase gradually with the occurrence and development of diabetic retinopathy in type 2 diabetes mellitus patients. However, the symptoms may be reduced in patients with severe DR.

ABSTRACT

Objective To observe and analyze the clinical features and prognosis of endogenous klebsiella pneumoniae endophthalmitis (EKPE).Methods This is a retrospective case series study.Seven patients (8 eyes) with EKPE were enrolled in this study.There were 3 males (4 eyes) and 4 females (4 eyes).The ages were from 39 to 76 years,the mean age was 57.29 years.All these cases had no history of trauma and surgery.Meanwhile,they all had some risk factors,such as infection,diabetes mellitus,systemic lupus erythematosus,liver abscess,renal insufficiency undergoing dialysis treatment,Hodgkin lymphoma and so on.All the eyes were undertaken visual acuity,slit lamp and fundus examination to observe the eye conditions.Seven eyes were undertaken pars plana vitrectomy with intravitreal injection of antibiotics from 2 days to 2 weeks after onset.And only one eye was undertaken intravitreal injection of antibiotics without surgery.Microbial stains and culture were performed for 7 eyes using vitreous and aqueous fluid samples from the procedures of vitrectomy.Meanwhile,culture and drug sensitive tests were performed from blood samples.According to the result of the drug sensitive tests,carbapenems such as imipenem and meropenem were used in each patient through intravenous injection from 1 to 2 weeks.During the follow up period from 3 days to 1 year,prognosis was observed at each office visit.Results From these eight eyes,presenting visual acuity was light perception (4 eyes),hand motion (3 eyes),0.1 (1 eye).Hypopyon (6 eyes),aqueous fluid opacity (2 eyes) and diffuse vitreous opacity (8 eyes) were found.Changes in fundus like optic disc,macular edema and retinal vascular occlusion could be observed.Cultures of the vitreous and aqueous fluid samples from vitrectomy were all point out to klebsiella pneumoniae.At last office visit,the visual acuity of patients with hypopyon was no light perception (1 eye),light perception (1 eye),hand motion (1 eye).The visual acuity of patients without hypopyon was 0.05 (1 eye) and 0.5(1 eye).Finally,1 eye was underwent enucleation and one patient with binocular disease was died of multiple organ failure.Conclusions EKPE is almost unilateral attacked.Changes in fundus like optic disc,macular edema and retinal vascular occlusion can be observed.EKPE is commonly associated with poor visual outcomes.It is useful to save patients' visual acuity by performing vitrectomy before hypopyon happened.

ABSTRACT

OBJECTIVE@#To systematically evaluate the clinical effect of cultured milk products as adjunctive therapy in the anti- Helicobacter pylori (H. pylori) treatment.
@*METHODS@#The randomized controlled trials (RCT) and Quasi-randomized controlled clinical trials (Quasi-RCT), which were used to evaluate the efficacy and safety of eradicating H. pylori by fermented milk-based routine treatment, were searched and collected in Pubmed, Embase, CNKI (China National Knowledge Infrastructure), CBM (Chinese BioMedical Literature Database), Wangfang Database, VIP (VIP Citation Database) from establishment of these database to February 2015. The combined relative risk (RR) of H. pylori eradication rate and the rate of side effects were analyzed. Sub-group and sensitivity analysis was performed, and the publication bias was also tested. 
@*RESULTS@#A total of 9 studies including 1 644 cases were identified. The H. pylori eradication rate was 79.5% in fermented milk products combined with routine therapy, and 67.0% in routine therapy. The combined RR of H. pylori eradication rate was 1.186 (95% CI 1.118-1.257), and the combined RR of total side effects was 0.706 (95% CI 0.373-1.340).
@*CONCLUSION@#Cultured milk products as adjunctive therapy is effective in improving the eradication rate during eradication therapy for H. pylori. However, it could not effectively decrease the risk of side effects.

?In allusion to the training requirements of eight-year program medical students, combining with our own experience in teaching this type of the students in ophthalmology, we have done some thinking about the training methods of eight-year program medical students in order to improving their comprehensive abilities of ophthalmology. Several suggestions are made in various aspects, including the study of the basic theory of ophthalmology, the training of doctor - patient communication skills, the training of basic clinical skills, the interest developments in ophthalmology subspecialty areas and the training of basic experiment skills.
